The BBGH Case Management team works in collaboration with medical providers, nursing staff and physical therapy, as well as with social workers and discharge planners from other healthcare institutions and agencies, to develop discharge plans with patients and their families to make arrangements for post-hospital care.
Supporting patients and family, as well as making sure patients have their basic needs met while they are hospitalized, may include arrangements for:
- Food
- Lodging
- Transportation
- Spiritual support
- Swing bed transfer
- Providing community resources
- Other needs
